Applying for Re-admission:
1. Fill out Application for Readmission Form, which is available at your Learning Center and Downloadable Forms.
2. Pay the application fee of P225. See payment options and procedure for local students.
3. Submit accomplished form and payment slip before the set deadline for the term.
4. Submit the documents to the LC Coordinator or email it directly to your respective Faculty Office.
5. Dean's Office through the concerned Program Chair acts on application (including request for waiver of MRR) and advises student in courses to enroll and revised plan of study.
6. The Office of the University Registrar (Records Section) informs student of the result of application approximately two (2) weeks before the start of the registration of the coming term.
