Step 2. Email scanned copies firstname.lastname@example.org or mail to Office of the Vice Chancellor for Academic Affairs (OVCAA), UP Open University Los Baños, Laguna 4031 the following documents:
Accomplished application form
photocopy of recent income tax return
Certification of employment and permit to study from your principal/ supervisor or head/president (Form 2)
Step 3. The Scholarship Committee evaluates all applications. An applicant should first be admitted to a degree program before the application for scholarship / study grant is evaluated.(see Criteria in Awarding Financial Assistance)
Step 4. The applicants will be notified of the official result of their application through postal mail and email. A list of qualified applicants is also sent to theLearning Centers.
Step 5. Qualified applicants are required to accomplish contracts for scholarship / study grant. Applicant should send back accomplished and notarized contract on or before the date specified in the contract to Office of the Vice Chancellor for Academic Affairs (OVCAA) UP Open University Los Baños, Laguna 4031
